Admissions Criteria

Applicants should…

  • Be deaf or hard of hearing and use, or be willing to learn, ASL
  • At least 21 years of age
  • Adults who have additional disabilities which preclude them from living independently
  • Demonstrate basic self-care (i.e.: not be in need of attendant services)
  • Document/demonstrate the willingness and ability to live in a group environment without being a danger to self or others
  • Have evidence/documentation/willingness to work
  • Family involvement and 3-6 month trial run

Referrals and Admissions

Individuals may be referred to KyADAPT from a variety of sources that may include, but not be limited to the following:

  • Self-referral
  • Family member or relative
  • Organization or agency (Comp Care, Vocational Rehab, etc.)
  • Other Independent Living program or Residential Care facility
  • Friend, neighbor or advocate

Regardless of the source of referral, the initial step in the admissions process is to complete the KyADAPT Initial Application Form. Upon receiving that completed and signed application, the admissions committee will determine if the applicant meets the KyADAPT Admissions Criteria. Applicants should be…

Once an individual is determined to meet the admission criteria, the KyADAPT Application for Services – Long Form will be sent to the applicant. The packet will also include the application form for the Danville Housing Authority and KyADAPT Release of Information form/s.

The KyADAPT Admissions committee will ensure that the applicant or parent/guardian has the ability to understand and complete the packet. If needed, a representative of KyADAPT, Comp Care, Social Services, Voc Rehab or other such agency will work directly with the applicant to complete the forms.

Upon receiving the signed Long Form application, the Admission Committee will select a person to review all information and make the needed contacts to obtain background reports and documentation. The committee will meet to review the collected data and make a decision regarding acceptance or not of the application.

Upon approval, the applicant will be notified of the decision.

Likewise, at any point in the admissions process, if an applicant is not accepted into the KyADAPT program, a letter signed by the Chairperson of the Board will be sent.

Transition

After a client has been accepted, and prior to moving into the program, the KyADAPT Client Services Consultant (CSC) and/or Admissions Coordinator will contact the client to set up the first Planning Meeting. The CSC/Admissions Coordinator and client, along with anyone the client invites, will initiate the transition process and start developing the intake information. Information will include the client’s:

  • goals, likes, preferences, needs, etc.
  • family members, friends and others who s/he wants to have involved
  • plans and resources for meeting financial needs
  • medical and counseling needs, and
  • any other information needed to plan for the delivery of independent living services.

Using this information, the CSC will identify a team of individuals who will come together to write the first Independent Living Plan and will attempt to ensure that all needed supports (housing, financial, staff, safety, accessibility, vocational, medical, etc) are in place prior to the client moving into the KyADAPT program.

During this transition process, the client will be invited to participate in any KyADAPT activities that the Planning group feels are appropriate.

At the point where the Planning Team feels that all needed supports are in place, a KyADAPT Admissions Agreement will be signed by all members of the group. The Admissions Agreement will outline KyADAPT and client rights and responsibilities.

At any point throughout the transition process, the client can decide not to join the KyADAPT program and withdraw their application. This must be in writing, signed and dated by the client. If at any point in the transition process the KyADAPT CSC feels the client and/or supports are not ready, placement will be put on hold until those concerns are resolved.

When KyADAPT services become available, we will contact you. At that time, we can discuss any questions you have as well as specific information about the services, medications, and individual needs. Until then, we hope you attend KyADAPT meetings and/or activities. You can keep up with us through the biannual newsletter mailed to all members.
